Applying to Five Branches (DAOM)

Once you have made the decision to enroll in the Postgraduate DAOM Program you are ready to begin the application process. With the MTCM or DTCM degree, you are also eligible to apply to our postgraduate DAOM program. To be considered for admission to Five Branches University, the following items are required:

Online Application (Recommended Method of Applying)

This is the online application form. Before you start, please prepare all the required materials. All materials must be submitted before completing the online application.

  1. Complete the application form in full.
  2. Portrait Photo – passport-style format.
  3. Statement of Purpose (one page) in PDF format or digital photo.
  4. Resume in PDF format or digital photo.
  5. Pay the application fee within the form: $100 (non-refundable, only payable via Visa, Mastercard, Discover, or American Express through PayPal).
  6. A copy of a government-issued ID (driver’s license or passport) in PDF format or digital photo.

    Please note: PayPal has temporarily stopped accepting international payments. If you are submitting the form from an international location, please jump to the Alternative Payment Exception section below. 

Alternative Payment Method (Not recommended; use only if submission of the online application is unsuccessful)

Please jump to the next steps if you have already submitted your online application form with the application fee. Otherwise, please indicate which program and campus you are applying to on a check or money order, and please mail it to your preferred campus. Or, please inform us if you are submitting your application using the fillable PDF (the Method of Exception). Download all PDF forms, complete them, and email them to [email protected]

The following required items will go through a document upload process and can be completed now or later, click here for the recommended method of submission:

Copy of Master’s or professional doctorate level diploma in TCM, Oriental Medicine, or Acupuncture 

(required only if applicable) 

Copy of valid malpractice insurance with Five Branches University listed as Additional Insured (required only if applicable).

Supporting documents-sent separately:

    1. Two Letters of Recommendation sent via email to daomadmissions@fivebranches.edu
    2. Official transcripts for all AOM and healthcare-related college/university coursework, enclosed in university-sealed envelopes and mailed directly to Five Branches University
